Title : Curious cases of refractory headache: Rare presentation of idiopathic intracranial hypertension without papilledema in tertiary care hospitals
Abstract:
Chronic refractory headache is challenging clinical entity because management is complex and unsastisfactory. Idiopathic intracranial hypertension is a disorder causing chronic refractory headache that results in isolated increase in intracranial pressure. Among aytpical presentation is iih without papilledema, unilateral presentation nad presentation with trigeminal neuralgia.we are presenting two cases of chronic refractory cases of chronic headache diagnosed as iih witout papilledema. Patient a 56-year-old with complaints of severe unilateral headache with mri finding and csf pressure suggestive of iih with neurointervention finding of negative transgradient pressure. Patient b presented with symptoms of unilateral trigeminal neuralgia and imaging findings with csf manometyry suggestive of iih with negative transgradient.both patients are discharged with acetozolamide and advised to follow up. Both are rare presentation of iih presenting as chronic refractive headache
